Player Cards

Risk Management Authorization (RMA)

Please remember: coaches and volunteers are required to obtain a RMA.  A volunteer is anyone who helps during practices, is a team coordinator, team referee, or helps the kids at any time during the season.  All coaches and volunteers should click here to register for a free RMA.

 

Player Cards

All Players (U6-U19) are required to have official player cards.  This is required to insure players on the field.  No players are allowed to play without a card present at the game.

How to create a player card:

  • You will need:
    • The official player card ("Member Pass")
    • Player picture
    • Laminating Sheet
    • Scissors
    • Glue stick
  • First separate the "Member Pass" from the "Membership Card."  The "Membership Card" should be handed to the player, it will not be used to make the official player card.

     
  • Next, attach the player picture to the back of the "Member Pass" card where it says "Attach Photo Here."  Also make sure the player has signed the card (note: if you forget or want to have them sign later, you can use a permanent pen after the card is made).

     
  • Next, cut a piece of your laminating sheet that is approximately 3 squares (3 inches) by the width of your laminating sheet (9 inches).  Then peel off part of the backing and place the card onto the sticky side of the laminating sheet.

     
  • Peel off the rest of the laminating sheet and fold over the other side of the "Member Pass" card.  Be careful not to create any bubbles.

     
  • Finally, cut off extra lamination material on all 4 sides of the player card (leave a small amount to help prevent the card from coming delaminated in the future).

     
  • Your player card is complete!
